The Referral Specialist will collaborate with other CAPS staff to provide continuity of care and follow-up to clients as they navigate services both on and off-campus.
The Referral Specialist will work closely with the Associate Director of Operations, Clinical Services Director in supporting clients as they receive services at CAPS in the coordination and management of the referral process to patients with health care needs that are not available within the normal scope of services of CAPS.
Responsible for providing direction for referral processes, including:
developing, maintaining, and monitoring referral lists and systems, and meeting directly with students in providing referral coordination and support.
The Referral Specialist will assist with coordinating follow up care including communication with clients, insurance companies, and community providers.
Collaborate with the Care Management Team in developing, maintaining, and updating information about communication resources and services.
This role is also responsible for communicating between CAP's, the University, students, and external providers.
This is a 10 month position with working hours while classes are in session.
What We're Looking For:
Required:
High School Diploma/GED.
4 years of experience in a medical facility, office administration, healthcare, mental health, social work, case management, or related field.
Demonstrated tact, patience and excellent oral/ written communication skills in dealing with patients, peers and external partners.
Ability to relate well to students, staff, faculty, and other constituents with the Ability to establish and maintain effective collaborative relationships with campus departments, student groups, and colleagues.
Strong interpersonal and customer services skills, including excellent communication skills (written, oral, and presentation).
Strong organizational and time management skills with the ability to complete multiple tasks in a busy environment are essential and prioritize work appropriately.
Effective analytical and problem solving skills.
Sensitivity to cultural and identity differences (broadly defined), with the ability to work well with a diverse student body.
Ability to perform effectively as a member of a multidisciplinary team.
Respecting and maintaining confidentiality.
Must demonstrate accuracy, proficiency, and efficiency in documentation of referrals.
Preferred:
Associates or Bachelor's Degree.
Experience working in electronic health record.
Knowledge of community-based resources and experience coordinating services, including experience with case management.
